RESTART_REQUIRED

Save active work, then explicitly run launch --restart. The CLI intentionally does not terminate an active Codex process without that flag.

Validation failed

Fix the reported pack files, rerun strict validation, and regenerate previews. Do not edit generated hashes by hand.
